Please complete this referral to request support for yourself (student) or a Mountain Shadows Middle School student. A copy of your form will be sent to the school counselor and wellness team.

THIS FORM IS NOT FOR EMERGENCIES: If this is an emergency, do not complete this form as your sole request for student support. If it is during regular school days/hours and the student is at risk of harming self or others, please contact your site administrator.
